Re-enrollment

Physical Exam Form
If your student needs a physical health exam (this includes all new students, kindergarteners, and seventh graders), you will need a printed form for your medical professional to complete. Download here or get copies from our office.
Authorization to Dispense Medication
This form needs to be completed and signed by a medical professional if your student will need medication during school hours, whether we administer the meds or the student does. Download here or get copies from our office.
